Low-density lipoprotein (LDL) and very-low-density lipoprotein (VLDL), which carry the ApoB protein, can become trapped in artery walls where they oxidize and trigger inflammation, leading to plaque formation and potential heart attacks. High-density lipoprotein (HDL), which carries ApoA, can exit artery walls without causing damage.
On the calorie debate, Attia clarifies that a calorie is not simply a calorie when discussing longevity and health outcomes. Two people eating 2,500 calories from different sources would likely have different health trajectories, energy expenditure, and quality of life, even if they achieved similar weight.
Attia emphasizes that longevity requires addressing emotional health alongside physical health. He uses thought experiments to illustrate that living longer without human connection, purpose, or self-respect would be a curse rather than a blessing. He dedicates the final chapter of his book to emotional health as one of three pillars of healthspan, alongside physical and cognitive health.
The "four horsemen of death" are: (1) atherosclerotic disease (heart disease and stroke), (2) cancer, (3) neurodegenerative diseases (Alzheimer's, Parkinson's, dementia), and (4) metabolic dysfunction (hyperinsulinemia, insulin resistance, fatty liver disease, type 2 diabetes). While the first three directly appear on death certificates, metabolic dysfunction acts as an amplifier, doubling the risk of the other three. Importantly, metabolic disease is causatively related to the other horsemen but is neither necessary nor sufficient—people can develop heart disease without metabolic disease, and not everyone with metabolic disease will succumb to these conditions.
Regarding cancer treatment, Attia notes that despite 50+ years of the "war on cancer," overall survival has only improved by 5-8%, with gains concentrated in specific cancers like leukemias and lymphomas. However, emerging immunotherapy and checkpoint inhibitors show promise by leveraging the immune system's ability to recognize cancer mutations. He emphasizes that detecting cancer earlier dramatically improves treatment odds.
On supplementation and the microbiome, Attia expresses skepticism about microbiome-based interventions, stating he has yet to see clinical examples where microbiome testing leads to actionable interventions that solve problems. While the microbiome clearly affects health, the "so what" question remains unanswered. He criticizes the supplement industry for lacking the rigorous clinical trial standards applied to pharmaceuticals, making it difficult to determine efficacy or safety.
Attia discusses his personal pharmaceutical regimen, which includes rapamycin (dosing once weekly for potential geroprotective effects), Repatha (a PCSK9 inhibitor for lipid lowering), and other lipid-lowering drugs to achieve physiologic ApoB levels. He takes these due to his family history of heart disease. He does not take metformin, as he does not have metabolic dysfunction.
On movement and stability, Attia explains that many injuries stem from poor breathing patterns, lack of segmental spinal control, and scapular dysfunction. He references two frameworks—Dynamic Neuromuscular Stabilization (DNS) and Postural Restoration Institute (PRI)—for retraining movement patterns. He notes that Tom's chronic trap injuries likely result from overcompensation due to weak mid-back musculature and poor spinal control.
When discussing life extension potential, Attia states that humans are unlikely to exceed the current longevity record of 122 years without technological breakthroughs. He believes the realistic gain from optimizing all factors is approximately 10 years, but emphasizes that the true value lies in healthspan—the quality of life during those added years. Most people who approach him about longevity actually mean healthspan when they think about it carefully.
