Neko's differentiator is longitudinal tracking—while a single dermatology visit might identify one concerning mole, Neko catalogs all 950 average moles and tracks changes year-over-year, something human doctors cannot reliably do from memory.
On systemic healthcare issues, Ek argues the fundamental problem is misaligned incentives. The healthcare system was built around acute infectious disease treatment, not prevention. Because health insurance is typically employment-based with average tenure of 2-3 years, insurers have little incentive to invest in preventative measures with 10-20 year payback periods. Neko aims to solve this by making preventative care so affordable ($500/year) that the ROI calculation changes for individuals and institutions.
Regarding AI and open vs. closed models, Ek advocates for both coexisting. He notes that while open-source models are valuable for innovation and cost efficiency, compute itself remains a defensibility mechanism—a model trained on 100,000 GPUs will likely outperform one run on a home computer, providing natural guardrails without needing to restrict open weights.
On Spotify's approach to podcasting standards, Ek confirms the platform supports both proprietary features and open standards, allowing podcasters to use Spotify's distribution tools to publish to other platforms via open standards. He committed to bringing feature requests (live support and value/donations through RSS) to the Spotify team.
Ek also recounted his pre-Spotify experience at Stardoll, a digital avatar site in Finland. Invited by investor Danny Reimer to help for 6-12 months, Ek re-architected the codebase, reducing page load times from 4 minutes to under a second, which caused traffic to explode and attracted Sequoia investment—but he left to focus on building Spotify.
